North Carolina - Upper Gastrointestinal Endoscopy LCD - (Revised)
Effective July 1, 2009, the LCDs for Upper GI for North Carolina (NC - L6686) has been revised to add the following ICD.9 codes as covered indication for Group 1 of the LCD.
