New Reopenings Request Form
In order to improve our current process and ensure your reopenings requests are processed in the timeliest manner possible, Part B Reopenings has created a new Reopenings Request form that will replace the existing form on the CGS Web site. The new form contains similar data elements but has a new format so that it's easier for you to identify, from the other forms you might submit to CGS. This form is still not required to complete a Reopening request but is very helpful in assisting us to correctly complete your request. The new form will be available by August 15, 2008.
Note: If you already have copies of the previous form printed, that's fine, because the existing form will continue to be accepted.
