Revision to LCD for Erythropoietin Analogs for Non-ESRD Use
An update to the Local Coverage Determination for ID, TN and NC for Erythropoietin Analogs for Non-ESRD Use has been made. The update corrects the placement of the explanation for ICD-9 codes with asterisks and adds an asterisk to code V58.11 with requirement for a secondary diagnosis.
