| Providers billing electronically for services with a -22 modifier should
put information in the notepad of the electronic claim to support the
request for additional payment. Under HIPAA rules, providers (other than
certain qualified small providers) cannot submit paper claims with documentation
at the time of initial submission. If sufficient information is available
on the claim to allow the reviewer to make an adjudication, the claim
will be processed. If the claim does not contain sufficient information,
the claim will be paid at the usual fee schedule allowable amount for
the base procedure. The provider may appeal for any additional fee, and
supply more supportive documentation at that time. |
