Please be aware that vendors, clearinghouses, and billing services offer various packages, which may or may not include the ability to utilize all of the features we offer. The following information will help you communicate your needs with your in-house programmers, or vendors and outside services.

Documentation Requirements
CIGNA Government Services requires certain documentation to ensure proper billing.
| Source Documents | Source documents are records verifying that the service billed was actually performed. Both the provider and CIGNA Government Services should review source documents in advance, and agree upon acceptable documentation. CMS specifies that source documents and medical records, pertaining to any particular Medicare claim, must be maintained for a minimum period of seven (7) years after the date the bill has been paid. |

| Beneficiary Signature Requirements | To simplify the transmission and processing
of electronic claims, providers can use one of two methods to
obtain the beneficiary's authorization:

| Medigap Signature Requirements | Payment information for assigned claims is forwarded to the Medigap insurers. A separate beneficiary authorization for Medigap assignment is required. A unique identifying number (the "OCNA") is assigned to these insurers for ease in electronic filing. |
| Provider Signature Requirements | When the EDI Enrollment Form is completed, provider signature requirements are met for electronic claims. |
| Educational Verification | CIGNA Government Services will contact you to assist you in meeting these documentation requirements. |
